• When self registering:

o Completing the Consent Form:

- You must have a witness sign the consent form.

- Fill out the time and date the form was completed.

- Indicate if you would like ashes returned. This can be changed at the time of death.

o Completing the Information required to complete a death certificate form:

- Make sure to completely fill out form. You can skip sections; 3, 4, 5a & 7.

- Put your legal next-of-kin’s information in the “Informant Information” section.

o Completing the Medical-Social History Form:

- Fill each section out with all current information

- We will revisit this form at the time of death to update information

• When registering someone besides yourself:

o Completing the Consent Form

- You must make sure you are the legal next-of-kin. In Illinois the legal next-of-kin is recognized in this order:

1) power of attorney for healthcare

2) surrogate decision maker identified by the attending physician

3) guardian of the decedent at time of death

4) spouse

5) adult son or daughter

6) either parent

7) adult sibling

8) adult grandchild

9) close friend

10) guardian of the estate

11) any other person authorized or under legal obligation to dispose of the body.

- You must have 2 witnesses sign the consent form.

- Fill out the time and date the form was completed.

- Indicate if you would like ashes returned. This can be changed at the time of death.

o Completing the Information required to complete a death certificate form:

All your information is held in the strictest of confidence and no information is shared with any other person or organization. When pre-registering, If all the paperwork is not completed in its entirety, the packet will be returned to the sender and the pre-registration process will be put on hold until all necessary documentation are completed in full. When the registration paperwork is complete we will send a letter with a copy of the consent and two wallet cards that indicate pre-registration with our program.
